Welcome Pack documents
Once you have registered with our service and have been contacted with an assessment date, you will need some documents for this assessment. This would either have been posted or emailed to you. With these documents you would have received a letter confirming your appointment date and time. It should also tell you whether it is by telephone or face-to-face. If for any reason you do not receive these documents on time or you have lost them, you are able to download a generic welcome pack below. If you are unable to print them, we recommend writing your answers on a piece of paper for ease.
